It is with a grateful heart that, at the invitation of your pastor or with his approval of your desire, you are about to begin the process of applying for Lay Formation. We have prayed for you when only God knew that you would wish to begin Lay Formation.
In order to apply for Lay Formation, you must first agree to a few requirements. Before we ask you if you agree to them, you need to know and understand them. In fairness to you and to ensure that your formation is complete, each participant in Lay Formation must first agree to do what is described on the Consent form before applying.
